Player Registration 2010-2011 Season 

Welcome New and Returning Players and their families to the Wesley Chapel Soccer Club!! We are excited that you will be a part of the 2010-2011 season.

We have provided this web page to help you organize the paperwork needed for the registration process.

Forms Needed

3)         Parent Financial Responsibility Agreement for the appropriate age group:

a.           U9 – U10

b.           U11 – U14

c.           U15 and up

4)         Parent Agreement Form

5)         Two original, notarized copies of the Medical Release form. A Notary will be available at the meeting if needed.

6)         Proof of Registration Payment (see registration instructions below).

7)         Player photo (Drivers License size) if one was not taken at the tryouts.

8)         Uniform Sizing should have been completed the last day of tryouts with the appropriate forms filled out and handed in. If player was not sized on the last day of tryouts please notify the Registration Table.
